Client Referral Form

If you are a case manager at a North Carolina prison or a North Carolina parole officer and are trying to secure a home plan for someone, please provide ALL of the information below. (You may need to answer these questions with the help of your client.) OurJourney and Crossroads Reentry will then work together to try to find a stable housing situation for your client.

Within a few days you will receive an email with several options based on the information you provide. You can then work with your client to decide which options to pursue. We recommend that you cast a wide net!

Timing is important! Your client should be no more than six months and no less than one month from his release date for best results. 


Please understand: We cannot guarantee that we will be successful in securing a place for your client to live, only that we are committed to pursuing all possible options. Either way, we will stay in contact with you throughout the process.

Is your client currently incarcerated?
Will your client need to register on North Carolina’s Sex Offender Registry upon release? Many places can’t or won’t accept those who have to register, but there are some places that will.
Is your client willing to go to Alcoholics Anonymous or Narcotics Anonymous on the outside? (It is five-times easier to place someone who is willing to attend either A/A or N/A.
Is your client an honorably discharged United States Military Veteran?
Has your client worked for at least 180 total days for Correction Enterprises during his incarceration?
Is your client able and willing to work? Clients who can’t or won’t work are the most difficult to place and typically end up in institutionalized housing.
